I ordered a certified copy of an SS-5 June 18 and it arrived at my office in
Canada Aug 10, 2010.
Perhaps yours is just lost in the mail. I suggest you send SSA a letter
(include dates of ph calls), ask the credit card company to refund the fee,
and then re-order.
